How do I create a form?

The Create New Form feature appears in the top portion of the Form & Survey Builder home page  (navigate to this page by selecting the related link from the TS Coordinator Survey area OR by choosing to add a Form to a DRF template as part of a requirement (for Authors to complete) or as an Evaluation method).

The remainder of the home page displays a list of your existing forms, which you can open to view, copy or edit.

To create a new form

On the Form & Survey Builder home page, enter a unique New form title in the box provided(maximum of 100 characters.
(Optional) Enter the Description/purpose of form (maximum of 2,000 characters).

If you decide to share the form, the contents of this description field will be visible to other TS Coordinators. It will not, however, be visible to people who are filling in the form.

(Optional) If you have created custom folders to organize your work in Taskstream, you can choose to file your new form in one of these folders by selecting the folder name from the Add to Folderpull-down list.
Click the Create New button. You are navigated to the Build Form page, where you can add newform elements and/or import elements from existing forms.

As you work, you can also move, copy or delete elements from your form.

(Optional) Click Preview to see how your form will appear to people who are asked to fill it in.
(Optional) When you have completed your new form, you may choose to share it with other TS Coordinators.

To create a new form:
1. On the Form & Survey Builder home page, enter a unique New Form Title in the box provided.
2. (Optional) Enter the Description/purpose of form.
3. Click on Create New.
